ChristianaCare HomeHealth is seeking a dedicated Registered Nurse (RN) to join our team. As a Field Nurse in Home Care, you will provide skilled nursing care to geriatric patients in the community, develop patient-centered care plans, and ensure patient well-being. Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
• Develop patient-centered care plans and ensure discharge planning.
• Maintain IV proficiency and perform IV functions efficiently.
• Schedule: Day shifts, one weekend a month, 2 holidays a year, and two on-call shifts a month.

Requirements

• BSN required; ADN candidates must obtain BSN within three years of start date.
• Current DE RN license or compact state license.
• 2 years of recent HomeHealth RN experience.
• Previous Home Healthcare and Med-Surgical experience preferred.
