Job Description
ChristianaCare HomeHealth is looking for a Full-Time Registered Nurse (RN) Field Nurse to join our dynamic team. In this role, you will provide skilled nursing care and treatment to geriatric patients in the community, develop and implement patient care plans, and work closely with our supportive leadership team to ensure the best possible patient outcomes.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Develop and implement goal-oriented, patient-centered care plans.
- Assist in the discharge planning process for patients.
- Attend to IV functions and be IV proficient.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of start date.
- Current DE RN license or compact state license.
- 2 years of recent HomeHealth RN experience.
- Previous Home Healthcare and Med-Surgical experience preferred.
