Registered Nurse (RN) - Home Health - $32-58 per hour

Job Description

Atrium Health is looking for a compassionate and dedicated Registered Nurse (RN) to join their Home Health team in Charlotte, North Carolina. In this role, you will serve as a case manager for adult and geriatric patients, providing direct patient care in the comfort of their homes. You will be responsible for overseeing post-acute services, coordinating care with a multidisciplinary team, and ensuring high-quality patient outcomes.

Responsibilities

• Manage an assigned patient caseload, ensuring timely care and documentation.
• Collaborate with a clinical team to address patient needs and goals.
• Supervise LPNs and HHAs in delivering care according to the established plan.
• Coordinate care with other healthcare providers to ensure continuity of care.

Requirements

• Graduate from an accredited School of Nursing, BSN preferred.
• Current RN license or temporary license in the state of work.
• Experience in home health care and case management preferred.
• BLS certification required.
