Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and rehabilitation potential.
• Provide direct patient care to address functional limitations and capabilities.
• Utilize EMR documentation for accurate record-keeping.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license in New York.
• Proficiency in CPR certification.
• Experience with EMR documentation is beneficial.
