Rehabilitation Therapist ($70-$90/hr)

Responsibilities

• Conduct home visit assessments for referred patients to evaluate medical conditions, functional capabilities, and rehabilitation potential.
• Provide direct patient care to support rehabilitation progress.
• Document patient information efficiently using EMR systems.

Requirements

• Graduation from an accredited Physical Therapy program.
• Valid and unencumbered Physical Therapy license in New York.
• Proficiency in EMR documentation (experience with Hello Note is a plus).
• CPR Certification.
