Rehabilitation Therapist ($70-$90/hr)

Join Paradigm Rehab as a Rehabilitation Therapist and make a difference in the lives of patients looking to reconnect with their environment and enhance their quality of life. As a key member of our team, you will conduct home visit assessments, provide direct patient care, and assess medical conditions to guide their rehabilitation journey.

Responsibilities

• Conduct Physical Therapy assessments during home visits.
• Assess patients' medical conditions, functional capabilities, and limitations.
• Develop and implement rehabilitation plans tailored to individual needs.
• Document patient progress in the EMR system.

Requirements

• Graduated from an accredited Physical Therapy college or university.
• Unencumbered Physical Therapy license for New York.
• Proficiency in EMR documentation (experience with Hello Note is a plus).
• CPR Certification.
