Rehabilitation Therapist Responsibilities
- Conduct home visit assessments to evaluate patients' medical conditions and rehabilitation potential.
- Provide direct patient care to enhance functional capabilities and well-being.
- Utilize EMR documentation efficiently to maintain accurate patient records.
- Collaborate with patients and families to develop comprehensive rehabilitation plans.
Rehabilitation Therapist Requirements
- Graduate from an accredited Physical Therapy program.
- Hold an unencumbered Physical Therapy license in New York.
- Demonstrate proficiency in conducting assessments and delivering patient care.
- Possess CPR Certification.
