Rehabilitation Therapist Requirements

• Graduation from an accredited Physical Therapy (PT) college or university.
• Valid, unrestricted Physical Therapy (PT) license for New York.
• Conduct in-home Physical Therapy assessments and deliver direct patient care.
• Proficiency in EMR documentation (experience with Hello Note is a plus).
• CPR Certification.
