Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as institutional living adjustments, financial concerns, family relationships, and end-of-life counseling.

Responsibilities
- Conduct psychosocial assessments.
- Provide counseling on various life challenges.
- Assist with discharge planning.
- Support patients and families through difficult times.
Requirements
- Bachelor's in Social Work (BSW), Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
