SOCIAL WORKER

Job Description

We are currently seeking a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients/residents and their families. Your role will involve addressing a variety of common challenges, including adjustment to institutional living, financial concerns, family dynamics, counseling related to medical diagnoses, aging-related changes, discharge planning, and end-of-life support.

Responsibilities

• Deliver clinical social work services and psychosocial assessments.
• Address a range of issues affecting patients and families.
• Provide counseling and support for emotional and practical challenges.
• Collaborate with interdisciplinary teams to ensure holistic patient care.

Requirements

• Bachelor's in Social Work (BSW); Master's degree preferred.
• LCSW-C certification preferred.
• LGSW certification.
• Prior experience in a healthcare setting is beneficial.
