Job Description

LifeBridge Health is seeking a dedicated Social Worker to join our team at Levindale Hospital in Baltimore, MD. As a full-time Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, and counseling related to end-of-life care.

Responsibilities

• Deliver clinical social work services to patients/residents and families.
• Assess and address various psychosocial issues including family relationships, powerlessness, and discharge planning.
• Collaborate with healthcare teams to provide comprehensive care.

Requirements

• Bachelor's in Social Work (BSW) required; Master's degree preferred.
• LCSW-C preferred.
• LGSW certification.
