SOCIAL WORKER

Job Description

We are looking for a compassionate and dedicated Social Worker to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges related to institutional living, family relationships, medical procedures, aging, discharge planning, and counseling.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients/residents and families.
• Assist individuals in coping with challenges such as financial concerns, family relationships, medical procedures, and aging.
• Collaborate with healthcare professionals to develop comprehensive care plans.
• Provide counseling and support related to end-of-life care and bereavement.

Requirements

• Bachelor's in Social Work (BSW) required; Master's degree preferred.
• Licensed Certified Social Worker - Clinical (LCSW-C) preferred.
• Licensed Graduate Social Worker (LGSW) certification.
• Demonstrated experience in providing social work services in a healthcare setting.
