Job Description
LifeBridge Health is looking for a compassionate and dedicated Social Worker to join our team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and families, addressing a variety of common challenges. This full-time position offers day shifts from 8:00am to 4:30pm in a supportive Allied Health environment.
Responsibilities
- Provide counseling and support to patients and families facing diverse social and emotional issues.
- Assess and address concerns related to institutional living, financial challenges, aging, and end-of-life planning.
- Collaborate with healthcare teams to develop effective discharge plans.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
