Job Description

We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health, a leading healthcare provider based in Baltimore, MD. As a Social Worker at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Offer clinical social work services to patients/residents and families.
• Assess and address various psychosocial issues affecting individuals and families.
• Provide counseling and support related to aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's in Social Work (BSW) required; Master's degree preferred.
• LCSW-C certification is preferred.
• LGSW certification is required.
