Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health's dynamic team. The role involves providing clinical social work services and psychosocial assessments to patients and their families, addressing a wide range of issues such as adjustment to institutional living, financial concerns, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services to patients/residents and families.
- Assess and address common problems related to various life situations.
- Offer counseling and support related to medical treatments and aging.
- Participate in discharge planning and empower patients in decision-making.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C preferred, LGSW certification required.
- Prior experience in a similar role will be advantageous.
