Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Provide counseling and support related to medical diagnoses, aging, and end-of-life care.
- Assist in discharge planning and address family dynamics.
- Collaborate with healthcare teams to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
- Experience in a clinical setting is a plus.
